  Application Forms 
 

Once you have an application form, please read it thoroughly, paying particular attention to our Terms and Conditions and the information about the account you have chosen. Please make sure that you provide all the information requested on your application form, as incomplete details will delay the processing of your application.

Notes
The Broadcasting Services Amendment (Online Services) Act 1999 and subsequent Internet Industry Associations Code of Practice prohibits the provision of Internet Access to anyone under the age of 18 years without obtaining consent from a parent or guardian. As such, it is imperative that a date of birth is provided on the Personal Account application form.

If you are under 18 years of age and wish to apply for an Alphalink personal account, please make sure you have one of your parents or guardians complete the consent section at the bottom of the application form.

If a date of birth is not supplied, or if you are under 18 and the consent section is not completed, your application will be delayed until this is provided.

You may fax your completed form (for credit card payments only), or mail your payment and form to any of the following locations:

Please make any cheques or money orders payable to Chariot.

Note: Please DO NOT both fax and mail the same application as you may receive, and be charged for, two accounts.

  Account Cancellation 
 

Accounts can only be cancelled upon receipt of a cancellation form. Cancellation forms can be obtained by contacting us on 1300 137 425

Technical Support for connection problems is free via telephone and email for all accounts. Support is available via phone during the following hours:

  • 8am - Midnight Monday to Friday
  • 9am - 9pm Weekends
  • 9am - 5pm Public Holidays

We only accept new applications by phone, post, fax or over the counter at our Offices. It is also possible to apply for some accounts online. Please click here for more details.
